Adult Foster Care


People, Incorporated offers Adult Foster Care (AFC) as part of its residential support network as an option for individuals who require limited assistance. Individuals in this program may need help with medical issues and/or daily living skills but wish to continue living an active lifestyle in the community. The AFC program creates a relationship between a family, the individual in need of services and People, Incorporated’s residential network that is beneficial to all.

The AFC program provides nursing care management and case management by a social worker to support the family, as well as the individual. The registered nurse and social worker schedule regular home visits to monitor the placement, as well as provide any necessary trainings the family may need. People, Incorporated also provides twenty-four hour emergency support.

Being able to enjoy the comforts and atmosphere of a private home helps the individual remain a part of their community while receiving needed supports from both People, Incorporated and the provider. The provider takes advantage of extra room in their house or apartment, coupled with their desire to make a difference in the life of someone with a disability. Caregivers also receive respite services from People, Incorporated that allow them to continue to have time away for a vacation or just a night out.
